Step by step guide:
- Click the green Sickness and unplanned absences button near the top of the page.
- Navigate to the Absence Records area in the lower half of the page.
- Identify the absence associated with the form you need to complete and press the Form button on its row.
- e-days will display the Self-certification form on the screen.
- Review the form and confirm its details are accurate by ticking the box in the Employee Declaration area of the form.
- Press the orange Submit button to submit your declaration.
