Step by step guide:
  1. Click the green Sickness and unplanned absences button near the top of the page.
  2. Navigate to the Absence Records area in the lower half of the page.
  3. Identify the absence associated with the form you need to complete and press the Form button on its row.
  4. e-days will display the Self-certification form on the screen.
  5. Review the form and confirm its details are accurate by ticking the box in the Employee Declaration area of the form.
  6. Press the orange Submit button to submit your declaration.
